• In-Person Visits: You can pick up the new client paperwork at our front desk. Feel free to ask our staff for assistance if you have any questions while completing the forms.

• Online Submission: If you prefer to complete the forms before your visit, you can download them from our website. Once filled out, you can bring the printed copies during your appointment.
